C:31:22 and :23 errors Sony miniDV - pinch roller?

I have a Sony DCR-PC101 that has been giving me c:31:22 errors (when a tape is NOT loaded) and c:31:23 error (when a tape is loaded). I find I can circumvent these errors by manually advancing a tape before inserting it, but this is obviously a less-than-ideal solution. I also have a pinch roller that comes off its spindle rather easily - the white/blue plastic inside the roller seems to be broken(?)

After these errors started, I went to these forums, and then tried the whack-a-Sony method, which didn't help. I also tried cleaning the roller, capstan pin, etc. - no dice.

My questions are about the pinch roller, since it seems that's where the problem is. Should the roller fit tightly on its spindle, so it can't spin independently? Right now my roller spins independently of the spindle. Also, does the roller fit all the way down on the spindle, so the top of the spindle is visible above the roller (that's how mine currently sits)? Or should there be a spacer at the bottom of the spindle that the roller sits on?
